Under the supervision of a Registered Nurse, the LPN/LVN (Licensed Practical/Vocational Nurse) – Home Health provides skilled one-on-one nursing care to patients in their home and accompanies them when indicated to appointments, etc.
Minimum Requirements:
- Graduate of an accredited practical or vocational nursing program
- Currently licensed as a Licensed Practical/Vocational Nurse in the state in which the LPN/LVN will practice
- One year prior professional nursing experience, home care experience preferred
- Complies with all relevant professional standards of practice
- Current CPR if applicable
- TB questionnaire, PPD or chest x-ray if applicable
- Current Health certificate (per contract or state regulation)
- Must meet all federal, state and local requirements
- Must be at least 18 years of age
