The RN Case Manager is responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime.

This position is a remote position. However, you must live within the county you are assigned to work... This position is for Kent County, DE. You must live within Kent County to be considered for the position. 

Minimum Requirements:

  • Current RN licensure in state practicing
  • At least one year of Case Management experience preferred
  • Current CPR if applicable
  • TB questionnaire, PPD or chest x-ray if applicable 
  • Current Health certificate (per contract or state regulation) 
  • Must meet all federal, state and local requirements 
  • Must be at least 18 years of age 
  • Remote Position - Must live within county you are working (Kent County, DE)
  • 6 weeks of training on technology and instruments
  • Schedule Mon-Friday 40 hours a week
