- Prior experience working with CalAIM and CalOptima Connect required
- Experience: =Housing experience required, with minimum 2+ years of CalAIM
- Community Supports or Enhanced Care Management experience managing a comparable
- or larger caseload w/proven track record of meeting documentation requirements.
- Technical: demonstrated success in housing deposits, navigation, tenancy sustaining, and
- day habilitation.
- Systems: HMIS experience required, CalOptima Connect experience preferred (or
- comparable payer portal).
Comprehensive Case Management: Conduct intake assessments, develop housing sustainability plans, monitor client progress, and provide mobile advocacy and crisis intervention using trauma-informed care.
Resource Coordination: Connect clients to essential services like Cal Fresh, General Relief, Cal Aim, disability programs, and community resources; follow up on weekly goals and action steps.
Documentation & Compliance: Maintain accurate case notes, input data and ensure confidentiality of client files.
Team Collaboration & Reporting: Participate in weekly case reviews with the Housing Sustainability Manager, attend team meetings, and report mandated abuse cases as required by law.
