Amergis Healthcare has a contract opportunity for a Case Manager to support individuals in an Addictions and Recovery setting in Danvers, MA.
Local Pay: $25/hour, paid weekly
Qualifications:
- Bachelor's Degree or equivalent, or Associate's Degree in a Human Services field
- 2-3 years experience and/or training in the area of chemical dependency
- Knowledge of relevant community resources for continuing care/aftercare placement
- Must be able to complete on site CPI and CPR training
- Must be able to work in a multicultural setting and maintain a customer service orientation
- Must be able to walk, stoop, kneel, and crouch. Must occasionally lift/move up to 20 pounds.
- Preferred: Certified Addictions Counselor (CAC)
- Preferred: Valid driver's license in good standing
Schedule: Part time and full time options available
- Sunday - Thursday 8am-4:30pm (40hrs)
- Sunday - Thursday 11:30am-8pm (40hrs)
- Wednesday - Saturday 10am-6:30pm (32hrs)
- Thursday - Saturday 10am-8pm (32hrs)
- Friday - Monday 10am-6:30pm (16hrs)
- Saturday/Sunday 9am-5:30pm (16hrs
Job Duties:
- Maintain an individual case load - intake evaluations, treatment and aftercare planning, medication management support, and safety planning as needed
- Relate to a substance abuse treatment population in a sensitive manner
- Complete all documentation in a timely manner
- Coordinate with collaterals and referral sources as needed
- Support other staff in milieu management, de-escalation, and crisis management
- Coordinate and support transportation needs to necessary appointments and aftercare
- Assess patients' continuing care needs, contacting appropriate aftercare resources as needed
Duration: 13 weeks with possible extension
